What to expect at your first visit to Sam's Family Dental

Thank you for visiting the website of Sam’s Family Dental. Our website provides important information that will help you become familiar with our practice. It also contains answers to frequently asked questions.

Please feel free to call us anytime.

We appreciate this opportunity to introduce ourselves and look forward to providing you with quality dental care.
We want you to know that in our office you can expect to be treated with respect at all times. We strive to deliver the best diagnosis and treatment that dentistry has to offer.
We believe that these goals can be met through a continuous and open dialogue between you and our entire team.
We encourage you to involve yourself in your own treatment and ask questions throughout our relationship. In our opinion, an informed and involved patient is most likely to achieve the highest level of healthcare, and we look forward to working with you to achieve this goal.
First Visit
Your initial appointment will consist of a comprehensive examination that will involve a review of your health and dental conditions. Typically this appointment involves:
Clinical Exam/Cancer screening
Charting existing conditions
Necessary X-rays
Review of your specific needs
After your examination we will explain your diagnosis and treatment options.
Please assist us by providing the following information at the time of your initial appointment:
X-rays from previous dentist (if applicable)

If you have x-rays from a previous dentist or physician, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
Our office uses digital x-rays, an advanced technique that reduces x-ray exposure dramatically. It is also easier to forward your digital x-rays via email should they need to be sent to a specialist or other practitioner.
A list of your current medications.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.)
If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.
Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on a separate day.
IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.

Scheduling

We will schedule your appointment as promptly as possible. If you have pain or an emergency situation, every attempt will be made to see you that day.
We try our best to stay on schedule to minimize your waiting. Due to the fact we provide many types of dental services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.
Please call with any questions or to schedule an appointment. Once your appointment is scheduled you can preregister with our office below. If you have any questions or concerns regarding this process please contact our office!
You may preregister with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the Submit button at the bottom to automatically send us your information.
On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.
